Why is S3 abnormal?

A S3 can be a normal finding in children, pregnant females, and well trained athletes, however a S4 heart sound is almost always abnormal.S3 can be normal in young (15-40 years) healthy people because the heart functions so well that the ventricles allow rapid filling. In older patients, it can indicate heart failure, as the ventricles and chordae are stiff and weak and reach their limit much faster than usual.
